We, the undersigned parent/guardian(s) of the named student(s) grant

permission for participation in this event, an event sponsored by River

Road Baptist Church. We have been advised of the nature and extent of

the activities that may take place and represent to you that the

participant is physically and mentally able to participate in these activities.

We understand that this activity, as in any activity for youth, does present

the risk of injury, or even death, and hold you, your agents, employees and

representatives harmless from any liability to any other person or entity

arising as a result of the conduct of the participants in this activity and

agree to defend and indemnify you, your agents, employees and

representatives against any claim or liability arising as a result of such

contact. If we, as parents or guardians are not personally present at these

activities in which the student is to participate, so as to be consulted in

the case of necessity, you are authorized on our behalf to arrange for

such medical and hospital treatment that you may deem advisable for the

health and well being of the participant. My child has insurance and is

under supervision of the church listed above. Activities may include the

following: swimming, boating, waterskiing, wakeboarding, innertubing,

paddleboarding, paintball, outdoor games, running, walking, jumping, hiking,

relays, overnight housing, competitions, campfire and other similar

activities.

I, as the student, understand that while attending River Road Baptist Church events,

I am under the direction and authority of the leaders in charge. These events

will be run within the standards set by River Road Baptist Church.

I understand that this is a Christian event and will have a spiritual

emphasis. I understand that I must register to attend and that

I am expected to be with my group at all times.

I also understand that the use

of alcoholic beverages, illegal drugs, tobacco, fireworks, weapons, foul

language and abusive or lewd behavior are prohibited. Any variance in the

rules and/or regulations will result in immediate expulsion; my parent,

guardian or emergency contact person will be contacted to pick me up.

I have read the entire release form, have discussed it with my parental

authority and agree to its contents.
